Notice | On-site acceptance of passport/travel permit applications in Phoenix on April 7 To facilitate local Chinese citizens’ application for documents, the Consulate General in Los Angeles will hold a ceremony on April 7 (Sunday…

In order to facilitate local Chinese citizens to apply for documents, the Consulate General in Los Angeles will send personnel to Arizona State University on April 7 (Saturday) to accept applications for passports and travel documents from Chinese citizens. The specific matters are now notified as follows:

> 1. Time and place

Time: April 7, 2018 (Saturday), 9:00 am – 4:00 pm.

Address: Neeb Hall, L.S. Neeb Hall, 920 S Forest Mall, Tempe, AZ 85281 (please click to view the map).

> 2. Scope of acceptance

> Subject to the constraints of venue, manpower and other conditions, this on-site visa application only accepts the following applications:

(1) Application for renewal of private passport with validity less than one year;

(2) Renewal of private passport with all blank visa pages Application;

(3) Application for lost passport reissue;

(4) Application for travel permit for Chinese children born in the United States.

3. Instructions for Applying for Certificates

(1) All applicants (including children) must carefully read and browse the "Instructions for Applying for Certificates from Other Places" published on our website, and must prepare all application materials item by item in advance as required.

(2) All applicants must make an appointment online and fill out the form in advance (please click here). This appointment for applying for the certificate is different from making an appointment at the Consulate General. Please do not select the place of application as "Phoenix City". The appointment system for on-site certification will open at 5:00 pm on March 20 and close at 5:00 pm on April 5 (Los Angeles time). Please make an appointment in time.

4. Notes

(1) The Consulate General accepts applications by appointment rather than on-site queuing. In order to avoid long waiting times and to maintain good order, please submit your application on time (it is not recommended to arrive in advance) according to the appointment time slot.

(2) The following situations will not be accepted:

1. Failure to make an online appointment, fill in and print the application form and other materials in advance;

2. Failure to select the "Phoenix City" as the required certification location when making an appointment;

3. Failure to prepare all the application materials as required;

4. Failure to truthfully declare the situation.

Our consulate will send personnel to Phoenix to apply for the certificate again in October this year. Please pay attention to arrange the time for applying for the certificate according to your personal situation.
